All orders with the “prescription” option selected will require a current prescription from your eye doctor that includes pupillary distance.  The pupillary distance is a measurement that your eye care professional should have on file and will be able to provide at no charge to you. You can email this form to orders@zzmedical.com or fax it to 319-277-3998. Please reference your order number when you send the prescription. We will not accept prescriptions that are not printed on a form provided by an eye care professional. Your order will not be processed until we receive your prescription form with the complete information.
